NOTE: This Annual Levy Return Form must be submitted no later than 5pm (AEST) on Friday, 10 September 2021.

Please enter your membership number along with some identifying information.

You can find your membership number and postcode in the email sent to you or on the hardcopy Annual Levy Return you will receive in the mail.

If your postcode is three digits, please include a leading zero - for example postcode 822 needs to be entered as 0822.

If you did not lodge an Annual Return in 2018/2019 or 2019/2020, you can do so after you’ve completed the Annual Levy Return for 2020/21 to maximise the votes allocated to you (for more information, see the FAQs):
